What is a Chronic Condition?

Chronic diseases are long-term health conditions that can have a significant impact on a person's quality of life. In order for patients to qualify for Chronic Care Management, the Centers for Medicare and Medicaid Services (CMS) require patients to have two or more chronic conditions.

In this video, Kathryn Anderton, Director of Clinical Services at ThoroughCare, explains what constitutes a condition as chronic, and how implementing care coordination can help patients become more engaged and motivated to self-manage their conditions. Identifying risk factors, promoting patient education and engagement, and providing medical reconciliation all contribute to the success of patients who are enrolled in a Chronic Care Management program.
